Dental and Vision Network Providers
Please click here to find dentists in the Guardian provider networks.  For the Brooklyn HealthWorks “Preferred” plan, search under DHMO/Pre-paid.  For the Brooklyn HealthWorks “Premier” plan, search under PPO.

Dental and Vision Enrollment Plan Options
Employees with Brooklyn HealthWorks medical coverage who also want dental and vision benefits must pick a plan at the time a group enrolls in Brooklyn HealthWorks, and can only make changes (including dropping dental/vision coverage) one time per year during the group’s annual renewal period.

Employees enrolling during the initial special open enrollment period (November 2 through December 15, 2009) will only be able to make changes during the group’s annual medical renewal period after at least 12 months has elapsed.  For example, if an employee enrolls in a dental/vision plan during December 2009, and the group’s medical annual renewal period is in February 2010, the employee will not be able to make a change to his or her dental/vision plan option until February 2011.
